Intrahealth Flu Vaccinations

Intrahealth will be coming into school on Wednesday 16th October 2024 to administer flu vaccinations to our children. Please complete and return the consent form to school before Friday 4th October. You need to complete both sides of the form and tick whether or not you give your consent. Thank you.
